| 1. What is an RLC
Assisted Living Centre or Retirement Home?
An assisted living facility is first and foremost a
home for seniors. Most homes of this type have private
apartments or bed sitting rooms, as well as a wealth
of common space such as dining rooms, exercise area's,
lounges, chapel, crafts room, doctors office, tuck shop,
Music room, Library etc..
Although many of the apartments and bed sitting
rooms have kitchenettes, the majority of the
meals are served in a public dining room, restaurant
style. |

2. What level
of care is available?
To be eligible to live in a Retirement Living Centre
assisted living facility we require that you live independently
with supervision. You will be required to be assessed
by our nursing staff to determine if your needs can be
met at one of our homes.

please refer to our Personal Care Package (PCP)
of that home. |

3. What services
are included in the monthly fee?
The services included for the monthly fee are explained
in detail in the PCP or Personal
Care Package this is also sometimes referred to
as the CHIP or Care Home Information Package. |

4. What level
of care can I expect?
All Retirement homes advertise they care for their residents.
The level of care will differ considerably from facility
to facility as will the cost. At our homes there is no
additional fee for help with medications. Minor medical
problems are expected and included in the basic rent.
If you have a question about a particular medical condition,
please do not hesitate to speak to one of our nursing
professionals. |

5. Is there
additional care available beyond the Personal Care Package
(PCP)?
As we age some residents will require additional care.
At this point our nursing staff will re-assess their
situation to see if they are still suitable as a resident
of the facility. If they are able to maintain their independence,
with assistance. We will propose an extended care package.
This Extended Care Package (ECP) is an additional cost
item. The cost will vary depending on assistance required. |

6. What
is the Empire Living Centre?
The Empire Living Centre is a retirement residence for
seniors who desire accommodations in a full-service community,
free from the traditional responsibilities of maintaining
a private residence. |

7. Must
I sign a lease?
You are not required to sign a traditional lease. However,
long term residents are required to enter a Financial
Agreement. This makes arrangements for the provision
of 30 days notice to the Centre before moving out. |

8. How
do I arrange to move?
After you have completed the Application for Residency,
a confidential interview will be arranged at your convenience.
We will provide information and advice on the various
options available to you, and together we will take care
of all the details. |

9. What
are your room rates?
Room rates vary based on room size and location.
There are a wide variety of floor plans for private accommodations available;
both small and large size bed-sitting rooms, as well as small, large
and executive one-bedroom suites.

10.
What is included in my rate?
Your room rate includes the following services:
- Three delicious meals daily; with two choices
- Coffee, tea and snacks; throughout the day
- The Wellness Centre (support/ exercise room)
- Dynamic social calendar
- Linen and towel provided and laundered
- Housekeeping services weekly
- 24 hour on-site attendants
- Visiting physician
- 24 hour security
- Private guest dining room
- Emergency call system
- Tuck shop on-site
- Chapel services
- Parking
Services Available (fee for service)
V.O.N. offer on-site personal care services. Enjoy other on-site services
including hairdresser/barber, foot care professional, esthetician & massage
therapist. Take advantage of the comfort and convenience of our guest
suites, catering service and personal laundry. |

11.
I am very active. What social programs can I expect?
Our Social Director has designed a dynamic social program
that reflects the needs of all residents.
You may select the activities that interest you the most. For example,
you may wish to participate in: shopping excursions, theme parties, bingo,
card tournaments, exercise classes, visiting entertainment, discussion
groups, arts and crafts, and much more.
Residents and family members are encouraged to submit recommendations
to the Social Director.
We would be more than happy to provide you with a copy of our Activity
Calendar and Newsletter. |

12.
What are the differences between Empire Living Centre & Empire
Terrace Suites?
Empire Terrace Suites offers its residents more independence
yet provides some services:
- All units equipped with:
1) Individually controlled
heat & air conditioning
2) Full kitchen complete with
three appliances
3) High-speed internet access
(fee for service)
4) Security system
5) Laundry facilities
Your package will include:
- One meal per day (supper)
full meal package optional
- Monthly housekeeping
- Access to all activities and events
- V.O.N. Services (fee for service)

13.
What about any medical concerns I may have or may develop?
Upon moving in to the Empire, our Health Care Liaison
will spend some time with you to complete your medical
profile. This is to ensure that all of our staff and
attendants are aware of any medical concerns you have.
Should any additional services be required at this time or in the future,
our Health Care Liaison will contact the C.C.A.C. (Community Care Access
Centre) for a needs assessment and to provide the best possible healthcare
professional to meet those needs.
We are also very fortunate to have the Victorian Order of Nurses of North
Bay (V.O.N.) whose offices are located
on-site. Their doors are always open to us for assistance in healthcare
issues. For more information regarding the V.O.N. and their services,
please refer to the enclosed brochure.
